The Diabetes Program is available to people with diabetes, pre-diabetes, pregnant with diabetes or at risk for development of diabetes. Certified Diabetes Educators (Registered Nurses and Registered Dietitians) are available to help manage, educate, and support individuals and families with diabetes. Services for diabetes education and management are focused on the needs of the individual after assessment by the Diabetes Team. We also provide timely access to diabetic foot care and wound care services.

Certified Diabetes Educator Services:
- Diabetes assessment and management across the lifespan
- Individual appointments to assess, monitor and manage diabetes or prediabetes
- Pregnancy management for T2 diabetes and Gestational Diabetes (GDM)
- Insulin starts and adjustments
- Medication starts and adjustments
- Nutrition assessments and care plans
- Child and family nutrition services
- Assistance with nutrition related issues: hypertension, lipids, weight loss, and other general issues as required
- Smoking Cessation Counselling/Support (TEACH Certified)
- Presentations/Education upon request
- Other: Healthy Lifestyles programming
Foot Care Services:
- Monofilament Testing
- Treatment of corns, calluses, thickened, fungal deformed nails
- Wart treatment
- Inlow’s Screening
- Referrals to Chiropodist
